Here’s how you can keep your information up to date:
Updating Personal Details (Name, Address, Date of Birth, NHS Number)
These updates must be made through your GP surgery
- Name changes: Provide supporting documents (e.g. marriage certificate)
- Address changes: Simply inform your GP of your new address.

You have a few options for updating your contact information:

• Open the app
• Go to Account and Settings
• Select Manage NHS Account
• Choose Contact Details
